Note: Review the PSO Packet Check off Sheet for all instructions and information for the enrollement packet. Include this completed Check List with the rest of the packet.

  1. Registration and Travel Authorization Form
  2. Vehicle Form #81
  3. Application is complete and signed with two references opened and
      reviewed. (NOTE: please have VISTA sign the certification page for any electronic     submissions of the application and submit it along with the application materials)     Candidates can complete the application on-line at:     (https://recruit.cns.gov/applyform.asp)
  4. Benefit Election Form
  5. Sponsor Evaluation Form
  6. Applicant has been interviewed by the Sponsor/Supervisor
  7. “What To Know Before Beginning Service” Form
  8. Proof of Citizenship or Lawful Permanent Residency (include copy of
      Identification documentation with form)
  9. Direct Deposit Form (can also be turned in at PSO)
  10.-12. WP, PSO, LA Checklist Confirmation
      • A copy of the VISTA Work Plan and Position Description signed by the applicant.     One copy sent along with application and one copy to be given to the applicant to     take to PSO.
      • Applicant knows the dates of the PSO he/she will be attending
      • Applicant knows the monthly subsistence allowance
  13. Will childcare be needed?
